New referrals are reviewed as promptly as possible, including evenings, weekends, and holidays. Urgent needs are prioritized according to the information available, patient location, and clinical circumstances.

Referral pathway

  2. Clinical reviewWe review the patient’s full clinical picture and clarify missing information.
  3. Family conversationWe explain hospice, answer questions, and confirm the patient’s goals and choice.
  4. Assessment and admissionWhen eligible and elected, we coordinate the assessment, plan, medications, equipment, and visits.
