- Ask us to contact you in a specific way or at a specific location.
- Ask us to limit what we use or share; some requests may not be required by law.
- Get a list of certain disclosures we made.
- Choose someone to act for you and receive a paper copy of this notice.
- File a complaint without retaliation if you believe your privacy rights were violated.
How we may use and share information
We may use and disclose health information for treatment, payment, and health-care operations; to coordinate your hospice care; and when permitted or required by law. Other uses and disclosures generally require your written authorization, which you may revoke in writing as allowed by law.
Additional protections
Certain records—including substance-use-disorder treatment records protected by federal Part 2 rules and other information protected by California or federal law—may have additional limits on use and disclosure. Charity follows the protections that apply to the information involved.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
